We are currently looking for a Team Leader within our Home Care division based out of our Tea Gardens office. You will provide day-to-day leadership and supervision to a team of geographically dispersed HomeCare Support Workers with the need to travel to Port Macquarie once a month for a couple of days. This is a critical role ensuring every client receives high-quality, individualised and safe care, aligned with their personal goals, while fostering strong engagement and performance within the support-worker team.

Key Responsibilities

  • Individualised Care: Build respectful relationships with clients, champion their dignity and preferences, and relay feedback that keeps support plans up to date
  • Integrated Care & Communication: Act as the conduit between corporate, clinical and field teams; host team meetings; keep accurate documentation; and model RSL LifeCare values
  • Effective Care & Quality: Oversee delivery of complex-care tasks, monitor client wellbeing, manage supplies, lead complaint resolution and support audit readiness
  • Safe Care & WHS: Drive a safety culture, ensure correct medication management, report hazards/incidents in Riskman and model sound professional boundaries
  • People Leadership: Buddy and mentor new staff, coordinate rosters, support performance reviews and assist with recruitment and disciplinary processes

Key Working Relationships
Home Care Support Workers (direct reports); HomeCare Area Manager; Care Coordinators; Registered Nurses; Workforce Planning; Clinical Governance; People & Culture; Risk & Compliance; WHS; plus clients and their families.

To be successful in this role we’d expect you to have:

  • A Certificate III in Individual Support (Ageing or Disability) and medication-assist module CHCC305C (or equivalent)
  • Proven community aged-care experience, including complex-care and medication support
  • Ability to consult and influence stakeholders; strong organisation, delegation and MS Office skills
  • Sound knowledge of Aged-Care Quality Standards, infection control, manual-handling and other safe-work practices
  • Compassion, empathy and commitment to client-centred care

It is a mandatory requirement for a team member working in the Aged Care sector to have a valid Police Check or a valid National Disability Insurance Scheme (NDIS) check, or potentially both, to undertake the inherent requirements of the role.
